To receive your refund, MAIL in the following:
- Original cash register receipt dated between 6/13/12 and 7/8/12 showing purchase of (1) single unit package of LYSOL Dual Action Wipes with date and purchase price circled (postmarked no later than 8/17/12).
- Original UPC symbol from the LYSOL Dual Action Wipes package
- The completed rebate form
